Here’s what needs to be included in your enrollment.

All of the forms required to enroll in the Corsana plan are available in the table below. Each enrollee must fill out our group enrollment form (Form 1) and the statement of health form (Form 2) if you wish to be considered for the Optimum level of coverage.


Already a plan member but looking to make changes to your coverage or personal information?

No problem. You'll find the change forms at the bottom of the table below.


Making Extended Health Care Benefit Claims?

All claims are processed by Green Shield Canada and claim forms should be sent directly to their offices. 

All claims inquiries should be made to Green Shield Canada Customer Service at 1.888.711.1119

The 60-day offer for is for enrollees who need to provide evidence that they are enrolling with 60 days of hire, retirement, full-time transfer, etc.

Corsana Group Enrollment Form

ALL enrollees must complete this form.

Statement of Health Form

Any enrollee wishing to be considered for our Optimum health plan must submit form 2 with their enrollment.

Personal Information Change Form

Any plan member looking to make changes and updates to their contact information or dependent/spousal coverage options

Coverage Change Form

Any plan member looking to make changes and updates to their plan coverage options
